Participant Agreement
PARTICIPANT AGREEMENT
1. I am aware of and understand the potential risks and dangers associated with this physical activity, ie. minor cuts, scrapes and possible bruising, etc. I confirm I am voluntarily participating in these activities with full knowledge of the potential risks and dangers involved.
2. I understand that activities in the outdoors involve a risk of more serious injury or even death. I confirm that I am voluntarily participating in these activities and with full knowledge of the dangers involved.
3. I know of no reason why I should not participate in any of the activities offered by Jurassic Watersports. I hereby declare, to the best of my knowledge that I am free of any condition, disease, infirmity or illness that may affect my ability to participate in any activities. I also confirm that I have provided information of any and all existing medical conditions or disabilities on the Medical Form on the reverse.
4. I agree to abide by all oral warnings and notices regarding safety whilst participating in a Jurassic Watersports Activity. I am aware I have the opportunity to ask questions about the activities. If I choose not to take the advice or to disregard any advice given, I do so voluntarily and accept liability for all resulting injuries or damage.
5. I do hereby waive, release and discharge Jurassic Watersports from any and all responsibility or liability for injuries or damages resulting from my failure to follow any commands, warnings or safety briefs from members of the Jurassic Watersports team, while participating in a Jurassic Watersports Instructor lead session.
6. Jurassic Watersports have the right to take photos and videos of me whilst on session to use on social media and website ect, unless I formally inform Jurassic Watersports in writing, prior to my session.
7. By signing I give Jurassic Watersports the right to contact me regarding my activities and to be added to the Jurassic Watersports mailing list.
8. I consent to Jurassic Watersports storing my personal information in accordance with British Data Protection and Insurance regulations
ACKNOWLEDEGMENT OF RISK
All activities operated by Jurassic Watersports are physically demanding adventurous outdoor pursuits that carry with them inherent risks of accidental injury. Our team works incredibly hard to minimise these risks as much as feasibly possible. However, these risks include serious physical harm, broken bones, cuts and lacerations, head injuries and drowning.
Expeditions can be cut short, cancelled or adapted at any time before or during the expedition if any one of our team feel the conditions on the ground/water are no longer appropriate or safe to operate in for the given participants. Any participant has the right to back out at any time if they feel unable to participate further. (Our team will of course try to encourage you where appropriate, but you should never feel under any pressure to participate)
It is important that we are made aware of any illnesses or injuries that may affect your ability or safety during an expedition. Please note, we ask for this information in no way to disqualify you from taking part, it is simply in everyone's best interest that we are aware of these conditions so that we can ensure your safety, and the safety of others.
Conditions that are of particular importance are:
- Asthma and other breathing conditions
- Heart or respiratory conditions (even if in the past)
- Physical injuries affecting or limiting movement/mobility
- Recently healed injuries or surgeries (Last 12 months)
- Ongoing Mental Health concerns
- If you are currently or recently Pregnant
Any other medical conditions requiring you to take medication, or that you would usually have to inform a health care practitioner about prior to treatment.
Accidents are always a possibility, following the directions of your instructor are key to mitigating the risks of an accident occurring. Jurassic Watersports can not accept responsibility or liability for any accident that occurs as a result of not following directions, or not as a result of negligence on the part of a member of our staff.
DofE Expeditions involve long periods of remotely supervised travel (Ie. participants will not have instructors with them at all times), camping on wild sites and campsites (sometimes without staff present), cooking on camp stoves and carrying heavy bags and equipment.
*If booking for a participant under the age of 18, it is important that all parents and legal guardians are informed and aware of the information above.
Cancelation Policy
Less than 7 days after booking:
We offer a 7 day cooling off period, so if you decide to cancel within 7 days of booking we will offer a full refund. (This is only if none of the following conditions apply)
Over 30 days before Expedition:
If you cancel your expedition more than 30 days before the expedition start date, we will do our best to either move your booking, or offer a refund. However, we may only be able to offer a refund of up to 50%.
Under 30 days until Expedition:
Any cancellations made less than 30 days before your Expedition start date are not eligible for a refund. This is because we would already have staff and campsites in place that will not be refundable. Cancellation this late could also mean the expedition needs to be cancelled, meaning we may have to compensate other participants. That being said, if it is possible, we will offer to move your booking.
